b11982：hexagon：支持 UPSCALE op（#31331）

hex-upscale：基础 upscale 算子（未完全向量化）；hex-upscale：优化 nearest 2x、4x 与 bilinear 4x；hex-upscale：支持非连续行与维度折叠；hex-upscale：用 vgather 取代标量中转；hex-upscale：将全部 kernel 选择逻辑移至 host 端，按评审意见修改并清理；tests：新增 permuted UPSCALE 测试，覆盖 qwen3-vl 图像到文本场景。
